Please complete this referral form with as much detail as possible. The information provided will enable us to assess whether Pink Academy can meet the young person’s educational, social, emotional and safeguarding needs.

Following receipt of this referral, our team will:

• Review all information provided.

• Contact the referrer if further information is required.

• Arrange an initial consultation (where appropriate).

• Complete an initial assessment.

• Confirm whether a placement can be offered.
